If someone meets the criteria for alcohol, cannabis, and cocaine abuse would that give them the diagnosis of poly-substance abuse because there are three substances, even though one is alcohol, or would they all be written separately without the poly-substance title and code?

If the person fulfills the contract of substance abuse — failing work related obligations, arrests, hazardous situations, interpersonal problems — and they abuse three or more drugs, then that’s poly-substance abuse.
